Diabetic Retina

Prof. Dr. Suhail Sarwar

What Is A Diabetic Retina?

Diabetic retina, also known as diabetic retinopathy is the development of a disease in the eye. The altered vision, in this case, is the result of high blood sugar levels.

The retina is the layer of tissue at the back of your eye. It lines the inside of your eye and helps you see.

When you develop a diabetic retina, high blood sugar levels damage blood vessels in your retina. Thus, can cause inflammation and scarring, which leads to vision loss as well as total blindness.

Now the question arises, how does it develop?

Diabetic retinopathy develops when blood glucose levels are too high for too long. These high blood glucose levels can occur because you have diabetes or because you have a high intake of food that contains high amounts of simple sugar.

The diabetic retina is a condition that mostly develops in people with diabetes in Pakistan, but it can also occur without diabetes. It is the leading cause of blindness in working-age adults in the United States.

Diabetic retinopathy develops when high blood sugar levels damage small blood vessels in the retina and cause them to leak fluid and become swollen or bulging.

The fluid can leak into surrounding tissues, including the macula (the center part of your vision), which can result in blurred vision and even blindness if not treated early on.

There are several options available for diabetic retina treatment which includes laser surgery, medications, etc.

Common diseases due to the diabetic retina:

Before exploring further into the diseases, here’s what you need to know, primarily to identify if you are at risk for the diabetic retina. Symptoms of diabetic retinopathy include:

  • Blurred or reduced vision, especially at night
  • Halos around lights
  • Double vision
In addition to causing vision loss, the diabetic retina can lead to other complications such as macular edema. Macular edema is a condition in which fluid builds up in the macula and causes vision problems.

The macula is the part of the retina that receives the highest concentration of light rays. It's responsible for central vision.

It means seeing what's directly in front of you is possible if you have a healthy macula.

If you have macular edema, you may experience blurry vision and other changes in your vision such as distortion of colors. Macular edema may also cause pain and swelling in the eye.

Treatment of macular edema is possible through eye drops and steroids. Moreover, laser therapy is a treatment option that can be used by destroying the blood vessels in the retina. If macular edema is severe enough, it may be necessary to have laser therapy performed on an outpatient basis.

The doctor may also suggest vitrectomy surgery for the treatment of macular edema, but it's very unlikely.
Treatment for macular edema is possible but, in case of chronic edema and late diagnosis, the damage to the retina becomes irreversible.

Another type of diabetic retina results in diabetic retinopathy. Diabetic retinopathy vision occurs when blood vessels in the retina of your eye are “damaged” by high blood sugar levels. To analyze the difference between the two types, focus on the word damage.

The damage can lead to blindness or other vision impairment if not treated early on. In order to know if you are being diagnosed right, know that diabetic retinopathy on fundoscopy is identifiable and mostly looks like cotton wool spots.

When it comes to diabetic retina treatment, there are the following available options in Pakistan:
  • Laser surgery to treat the growth of new blood vessels and associating problems linked with diabetic retinopathy
  • Eye injections to stop the damage from exceeding and minimize the instant threat to the vision
  • Eye surgery can also be performed to remove blood tissues causing the disturbance. Moreover, if the laser cannot be applied retinopathy is a new phenomenon.
  • Treatment may also include taking medications to lower blood pressure and cholesterol levels or surgery such as laser photocoagulation therapy or vitrectomy surgery
Another type of diabetic retinopathy involves damage to cells that line the blood vessels (the retina). The most common types of damage are called proliferative diabetic retinopathy and geographic atrophy.

Treatment for diabetic retinas on the whole has side effects that last for only a few hours. Here's what to expect if you are planning to go for laser or surgical treatment.
  • Blurred vision
  • Increases sensitivity
  • Aches and discomfort
However, if the symptoms persist longer than usual, make sure to get in touch with the doctor, before it may get worse!
